Salary and Benefits

“BENEFITS AT A GLANCE”

HEALTH BENEFITS:
      The organization pays 100% of Life insurance premiums and contributes to your Medical, Dental and Vision premiums. Your dependent coverage is subsidized by the organization.

MEDICAL
      AmeriBen/IEC Group administers our plan and Blue Cross/Blue Shield provides the PPO network. The plan allows you to visit any participating physician. Coverage through a participating physician is payable at 100% after your $15 co-payment. You may visit a specialist without a referral. The individual lifetime maximum is $2,000,000.
      You may choose to visit any Out-Of-Network physician. With this plan the individual deductible is $500 and the family deductible is $1,500. The plan then will pay 60% of eligible expenses.
      The plan offers mental health coverage, wellness benefits and pharmacy coverage.
      The co-payment for outpatient Laboratory, Radiology and Physical Therapy Services is waived if performed at Casa Grande Regional Medical Center. Deductibles for inpatient care, outpatient procedures and OB delivery are also waived if at Casa Grande Regional Medical Center.

DENTAL
      Principal Financial Group provides dental coverage. This is an indemnity dental plan. Preventative and diagnostic (exams, cleaning, fluoride treatments and x-rays) are paid at 100% of usual and customary, with no deductible. Basic Services (fillings, extractions, endodontics and periodontics) are paid at 80% with a $50 deductible. Major Services (crowns, bridges and dentures) are paid at 50% with a $50 deductible. The maximum family deductible is $150 per calendar year and the maximum benefit is $2,000 per person per calendar year. Orthodontics is covered for children with a $2,000 lifetime maximum.

VISION
      Vision Service Plan (VSP) provides vision coverage. This is a Preferred Provider Organization that has over 21,000 member doctors nationwide. You may use a VSP member doctor in any of the 50 states. There is a small co-payment for the different services offered. There is also a maximum allowance for each of these services.

LIFE INSURANCE
      We offer life insurance benefit for our employees and their dependents through Mutual of Omaha. The employer pays 100% of the premiums for employees. Employees have $50,000 coverage, and may cover their Spouse for $5,000 and Dependents for $2,000 at a minimum cost.
FLEXIBLE SPENDING ACCOUNTS
      The corporation also makes available to you the employees the option of participating in the Section 125 Cafeteria Plan Flexible Spending Accounts.

LONG TERM DISABILITY INSURANCE
      After one year of employment, full-time employees are provided long-term disability coverage from CNA Insurance Company. This coverage provides 60% of the employee’s salary after they have been permanently disabled for three months or more. This coverage is paid 100% by the corporation.

SUPPLEMENTAL INSURANCE
      The corporation provides options for the employee to purchase supplemental life, accident, disability, cancer, and intensive care from American Family Life Assurance Company (AFLAC). This coverage is also available on pre-tax basis.

EMPLOYEE ASSISTANCE PROGRAM
      The National Employees Assistance Program (NEAS) provides the Employee Assistance Program (EAP). NEAS is available in all 50 states. This service provides all employees and dependents free confidential counseling services 24 hours a day, seven days a week via a 1-800 number as an added benefit. This service is provided to you free of charge.

TUITION REIMBURSEMENT
      After ninety days of employment, full-time employees are eligible to receive tuition reimbursement for additional education. This reimbursement may be used for individual courses, certification programs, continuing education credits, or degree programs. Non-degree education or undergraduate programs are reimbursed up to $5,000 annually, and Master’s degree program is reimbursed up to $7,500 annually. One hundred percent reimbursement is paid for an “A” or “B” and 75% for a “C”.

PAID TIME OFF
      Paid Time Off (PTO) is accrued on a pay period basis. During the first four years of employment you will accrue 160 hours. From the fifth through ninth year you will accrue 200 hours and the tenth year onward you will accrue 223 hours. This time can be used for paid time off, holidays and illness not covered by Major Medical. Employees are required to use 80 hours per year of PTO.

MAJOR MEDICAL
      We know how important financial protection is to you and your family, we provide major medical bank is for extended illness, surgery, etc., and may be accessed on the fourth day of illness. It also accrues on a pay period basis with a cap of 480 hours, or 12 weeks. From your date of hire you accrue 120 hours or three weeks per year. You are eligible to use major medical after 90 days of employment.

HOLIDAYS
      The Corporation recognizes Memorial Day, July 4th, Labor Day, Thanksgiving and the day after Thanksgiving, Christmas Day and New Years Day as approved holidays. Employees may use PTO for time away from work on these days.

403(b) PROFIT SHARING PLAN
      All employees meeting the eligibility requirements may participate in the 403(b) profit sharing plan. The organization will match 50% of the first 5% of the pay you contribute to the plan through salary deferral.

BEREAVEMENT LEAVE
      All full-time and part-time employees are allowed 3 days paid time off for death of an immediate family member.

JURY DUTY
      All full-time and part-time employees are allowed time off for jury duty service. The corporation reimburses the employee for the time away from work, up to twenty days per calendar year.

CREDIT UNIONS
      Pinal County Federal Credit Union, First American Credit Union, First Federal Credit Union, and Arizona Central Credit Union provide services for employees. The credit unions offer checking, savings and discounted loan options to employees.

EMPLOYEE HOSPITAL DISCOUNTS
      Employees are given a 10% discount for items purchased in the gift shop. They are also given discounts for all meals purchased in the cafeteria. On the employee’s birthday, they are given a coupon for a free meal in the cafeteria, free dozen cookies, or an additional 15% discount in the gift shop.

ENTERTAINMENT DISCOUNTS
      Discount coupons are available for Six Flags, Knotts Berry Farm, Universal Studios, Sea World, and surrounding San Diego attractions.
